Below are downloadable forms you will need for us to begin working together. You may not have to fill out each one. Please consult with Emery to discern which forms you will need to download and fill out.

After you fill out a form, you can save it, and email it to us, or simply bring it to your next appointment. If you are filling out a printed form, feel free to scan it, or take iPhone photos of each page. Those can be emailed to us at emeryerhart@ppfcounseling.com .
